FINANCIAL ASSISTANCE POLICY

Financial Assistance Policy:

                It is the policy of Dakota Gold Soccer Club to provide the opportunity to play youth soccer regardless of the ability to pay the established fees. Those unable to pay a fee may be awarded assistance. The amount of assistance provided to an individual or family will be based on their documented ability to pay and Dakota Gold’s ability to fund the amount requested. A reason to award assistance is solely the prerogative of the Dakota Gold Soccer Club.

Eligibility:

1.        Assistance will be granted on the basis of financial need. Eligibility criteria are based on the family income guidelines used by the Sioux Falls School District for the school lunch program. Those applicants for financial assistance for Youth memberships and programs will be required to provide a signature from a parent or guardian to verify eligibility. All memberships and/or program applicants will be required to provide a copy of their most recent pay stub and/or any copies of other financial support, such as: FDC, SSI, general assistance, etc

2. Applicants must provide an AGENCY REFERRAL.
3. Applicants have an opportunity to provide information on any extraordinary expenses, such as: Payments on school loans, out-of-pocket medical expenses, and/or situations, which may affect their eligibility for financial assistance.

Application Process:

1.        Financial Assistance Request Forms are available at the Dakota Gold office or on our website, clink in the link in the left menu. Adobe PDF Reader is necessary to view and print this document.

2. Request forms must be completed thoroughly and accurately, signed and dated.
3. Completed forms and required income documentation, etc. must be turned in to the Dakota Gold Soccer office.
4. Applicants will be notified if any financial assistance will be provided by Dakota Gold Soccer Club within two weeks of the date the completed form and required income documentation are turned in to the Dakota Gold office.
5. All financial assistance request records will be kept confidential.

                Financial Assistance is made possible by charitable contributions to Dakota Gold Soccer Club through generous friends throughout the community.
